The Mass Commanders, LLC. Basketball Program, the organizers, sponsors, volunteers, and players assume no responsibility for injury to the child named above. All rights and claims are waived by the undersigned parent and guardian. 

The undersigned parent or guardian gives approval for the participation of their child in all activities of the Mass Commanders, LLC Programs during the current season and assume all risks and hazard incidental to the conduct of the activities and transportation to and from the activities. 

Mass Commanders, LLC Camps complies with the regulations set forth by the Massachusetts Board of Health. Each camp location is individually licensed by each local Board of Health.

I herby grant permission to Mass Commanders, LLC and its entities to use my photograph or video footage of me or the photograph or video footage of my child(ren) in any printed publication, on websites, or in any related media story without further consideration. I acknowledge the Mass Commanders, LLC and its entities right to crop or treat the photograph or video footage at its discretion. I also acknowledge that the Mass Commanders, LLC and its entities may choose not to use my child(ren) in photo or video footage at this time, but may do so at its own discretion at a later date.

I also understand that once my image or my child’s image is posted online, any computer user can download the image.

All outstanding payments that are not made by the end of the season etc. (Fall – Nov. 14, Winter – Mar. 1st, and Spring – Jun. 29.) will be automatically billed via the stored credit card and/or debt collection via our lawyers.

A player that chooses to leave the program that has started a payment plan will be billed 40% of the remaining balance owed via the stored credit card or debt collection via our lawyer.

I agree to indemnify and hold harmless from any claims against Mass Commanders, LLC its entities and all employees related to the subject matter hereof.

Mass Commanders, LLC reserves the right to discontinue use of photos or video footage without notice.
